Healthcare Providers: To strengthen security and protect sensitive information, CMS is updating its MFT/UFM system. Effective October 26, 2026, nonregistered users will no longer be permitted to access the system or receive medical record requests through this process. Submit medical records for quality-of-care complaints, sanctions, and referrals through the new medical record submission portal. A CCN or NPI is required for verification. The process for appeals remains the same.
