Effective October 26, 2026, nonregistered users will no longer be permitted to access the system or receive medical record requests through this process. Submit medical records for quality-of-care complaints, sanctions, and referrals through the new medical record submission portal. A CCN or NPI is required for verification.

Healthcare Providers: The Centers for Medicare & Medicaid Services (CMS) is updating the Managed File Transfer (MFT)/Unified File Management (UFM) system. Beginning October 26, 2026, the system will no longer allow medical record requests to be sent. A new portal on our medical record submission page is available for quality of care complaints, sanctions, and referrals. A CMS Certification Number (CCN) or National Provider Identifier (NPI) will be needed for verification.
